Brock Whittaker c9514d0ad2 /help/: Remove underline and changes text from "Index" => "Main Index".
This removes the underline on hover and changes the text to get to the
index.

This also changes it to an <h2> tag so that it will be more inline
with the styling of the rest of the sections, but without the
down chevron.

Steve Howell aae0b2a826 Notify offline users about edited stream messages.
We now do push notifications and missed message emails
for offline users who are subscribed to the stream for
a message that has been edited, but we short circuit
the offline-notification logic for any user who presumably
would have already received a notification on the original
message.

This effectively boils down to sending notifications to newly
mentioned users. The motivating use case here is that you
forget to mention somebody in a message, and then you edit
the message to mention the person. If they are offline, they
will now get pushed notifications and missed message emails,
with some minor caveats.

We try to mostly use the same techniques here as the
send-message code path, and we share common code with the
send-message path once we get to the Tornado layer and call
maybe_enqueue_notifications.

The major places where we differ are in a function called
maybe_enqueue_notifications_for_message_update, and the top
of that function short circuits a bunch of cases where we
can mostly assume that the original message had an offline
notification.

We can expect a couple changes in the future:

    * Requirements may change here, and it might make sense
to send offline notifications on the update side even
in circumstances where the original message had a
      notification.

    * We may track more notifications in a DB model, which
      may simplify our short-circuit logic.

In the view/action layer, we already had two separate codepaths
for send-message and update-message, but this mostly echoes
what the send-message path does in terms of collecting data
about recipients.

Postgres doesn't like them, we don't have an obvious way to escape

the user to get an error.

Steve Howell c5b7e3f1ab Eliminate have_scrolled_away_from_top boolean.
The `have_scrolled_away_from_top` logic goes way back
to November 2012.

Now we unconditionally load older messages when we scroll
to the top of the feed. Before this bug, you could get
"stuck." It was a bit difficult to reproduce, but with
the right combination of render window sizes and batch
sizes, you would hit the home key quickly and hit the top
of the feed in a way that the flag got in the way of
going back in history.

Steve Howell 4d00c390ea Call maybe_rerender when prepending messages to a list.
We had a bug where once you scrolled back far enough
in the message view, your "window" for rendered messages
would be at the max, and `prepend` was not adjusting
the window correctly. Now we follow the example of
`append` and call `maybe_rerender`.

This partially addresses #6628, where users were
reporting that the home key stopped going up in their
feed. There was another bug at play for that issue
as well, which is fixed in the next commit.

Steve Howell 860f62c792 Have home key go to actual first message in the list.
Before this, the home key would go to the first message in our
render window. Now we go to the first message in our local
list of messages. (Note that there may still be older
messages, so it will still often take multiple uses of the
home key to truly get to the top of your feed.)

Some email services will provide default styling for things like
buttons, and in this case the site created a style that had a

because of CSS importance heirarchy. This swiches us to use
`background` as well.

This also forces no `text-shadow` on buttons.
